Anyone can make a referral, you can do it for yourself or it can be completed by a friend, relative, Doctor, Carer or Community Nurse on your behalf.

To be eligible to apply the person must be

• Living independently within the Prestonpans community
• Not a current resident of a Care Home
• They, their carer or family require extra assistance to maintain their independence

If you think you or someone you know may benefit from attending the Centre and they fit the above crieria then please download and complete the attached form as best you can and return it to us at the address below.

If you have any issues with opening or downloading the form or if you wish some assistance in completing it please do not hesitate to get in touch with a member of staff on the telephone number or e-mail address below.

Forms are also available for collection from the Day Centre and a member of staff will be happy to discuss any queries you may have.

We are currently registered to cater for 16 clients per day and there may be a waiting list which is addressed on a priority needs basis but you will be notified of the current situation once we have assessed your application.

If you are allocated a space then you will be invited to spend some time at the Centre meeting the staff and clients prior to starting. At Harlawhill we understand that joining an established group can be daunting and take some time to get used to. You can come with a friend or relative even just for a morning or afternoon and see what we do, we promise we are a friendly bunch and we will do our best to make you feel welcome.
